I, the undersigned parent/legal guardian, request voluntary participation for my minor (hereinafter referred to as "Minor") to participate in all aspects of the St. Norbert College Summer Band Camp
June 15-19, 2026, at St. Norbert College, which is hereinafter referred to as the "Activity".
This document includes the following clauses and will hereinafter be referred to as the “Participation Agreement”: (1) consent to participate in the “Activity”, (2) certification of health status and insurance coverage, (3) release for media/press coverage, (4) standard of conduct, (5) assumption of risks, (6) waiver of liability and indemnity agreement, (7) severability, (8) electronic signature agreement, (9) acknowledgement of understanding, and (10) acceptance of participation agreement.
CONSENT: I consent to “Minor”s participation in the "Activity" and acknowledge that “Minor” and I fully understand “Minor”s participation may involve risk of serious injury or death, including losses which may result not only from “Minor”s own actions, inactions or negligence, but also from the actions, inactions, or negligence of others, the condition of the facilities, equipment, or areas where the "Activity" are being conducted, and/or the rules for this type of event or "Activity". I understand that if I have any concerns regarding risk, I should discuss those concerns with the coordinators and staff of the “Activity” before I sign this agreement and throughout “Minor”s participation as needed.
CERTIFICATION OF HEALTH STATUS AND INSURANCE COVERAGE: I certify that “Minor” is in good health and has no physical condition that would prevent participation in the "Activity". Furthermore, I agree to use “Minor”s personal medical insurance as a primary medical coverage payment if accident or injury occurs. I consent to emergency medical treatment for “Minor” in the event such care is required, and I acknowledge that I am responsible for all related costs.
I authorize but to not obligate St. Norbert College, Inc. to provide emergency medical treatment for “Minor” in the event of an accident or illness that occurs while participating in the “Activity”. I assume “Minor”s responsibility of physical fitness and capabilities to perform the activities described and attest that “Minor” is physically able to engage in said “Activity”.
RELEASE FOR MEDIA/PRESS COVERAGE: I agree that photographs, pictures, slides, movies, video, or other media coverage of “Minor” may be taken in connection with “Minor”s participation in the "Activity" without compensation from St. Norbert College, the Premonstratensian Fathers, their officers, employees, and agents thereof and consent to the use of photographs, pictures, slides, movies, videos, or other media coverage for any legal purpose.
STANDARD OF CONDUCT: As a participant in the “Activity”, “Minor” agrees to adhere to the standards of conduct implemented by St. Norbert College, Inc. “Minor” is required to (1) listen and follow directions from directors and other adults, (2) always treat fellow musicians with respect, (3) and wait for parent/guardian to come inside for pick up and drop off. Unacceptable behavior includes, but is not limited to, (1) use of inappropriate language, (2) bullying, (3) and causing distractions that interrupt rehearsals or other activities.
ASSUMPTION OF RISKS: Participation in the "Activity" can be dangerous and carries with it certain inherent risks, such as but not limited to, being hit or struck by equipment, falling, contact during fall with the stage floor and the ground that cannot be eliminated regardless of the care taken to avoid injuries. Although the risk of injury is low during the “Activity”, there are still risks. These risks range from, but are not limited to (1) minor injuries such as slips, falls, cuts, scratches, bruises and sprains, (2) major injuries such as eye, joint or back injuries, fractures, concussions, heart attacks, and heat stroke, (3) catastrophic, life-altering injuries including paralysis, to (4) death.
ASSUMPTION OF RISKS (CONT.): I freely accept and fully assume all such risks, dangers and hazards and the possibility of personal injury, death, permanent disability, property damage or loss resulting thereof. Knowing and understanding the risks involved with participation in the “Activity”, I hereby voluntarily and willingly assume responsibility for all risks and dangers associated with “Minor”s participation in the "Activity".
WAIVER/INDEMNITY: In consideration of “Minor”s participation in the "Activity", I hereby waive all claims or causes of action against St. Norbert College, Inc., the Premonstratensian Fathers, their officers, employees and agents thereof, arising out of “Minor”s participation in the “Activity” and hereby release, hold harmless, and discharge St. Norbert College, Inc., the Premonstratensian Fathers, their officers, employees and agents thereof from all liability in connection therewith except such loss or damage which was caused by the sole gross negligence or willful misconduct of St. Norbert College, Inc., the Premonstratensian Fathers, their officers, employees, and the agents thereof. I agree I am financially responsible for any losses resulting from “Minor”s actions and will indemnify St. Norbert College, Inc., the Premonstratensian Fathers, their officers, employees and agents thereof, for any loss or damage caused by “Minor” during the "Activity".
SEVERABILITY: I, the undersigned, further expressly agrees that the foregoing “Participation Agreement” and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of the State of Wisconsin and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
ELECTRONIC SIGNATURE: By checking the "Accept Participation Agreement" box in the online registration process and typing my name into the signature box, I agree that the act of checking the box and typing my name not only constitutes my acceptance of the agreement, but is also my electronic signature and is the legally binding equivalent to my handwritten signature. Whenever I execute an electronic signature, it has the same validity and meaning as my handwritten signature. I will not, at any time in the future, repudiate the meaning of my electronic signature or claim that my electronic signature is not legally binding.
ACKNOWLEDGEMENT OF UNDERSTANDING: I have read this agreement and understand the terms used in it and their legal significance. This “Participation Agreement” is freely and voluntarily signed with the understanding that the right to legal recourse against St. Norbert College, Inc., the Premonstratensian Fathers, their officers, employees and agents thereof is knowingly given up, except for such loss or damage which was caused by the sole gross negligence or willful misconduct of St. Norbert College, Inc., in return for allowing “Minor”s participation in the "Activity". By checking the acceptance box and typing my name in the signature line on the online registration form, in lieu of my handwritten signature and in order to complete the online registration, this “Participation Agreement” is intended to bind not only myself but also my successors, heirs, representatives, administrators, and assigns.
ACCEPTANCE OF PARTICIPATION AGREEMENT: By checking the acceptance box and typing my name in the signature box on the online registration form, I attest I have read the “Participation Agreement” for the St. Norbert Collee Summer Band Camp occurring on June 15-19 2026, in its entirety and agree to all terms and conditions detailed in the “Participation Agreement”. I, as parent/legal guardian, do hereby give my consent for the minor(s) I have submitted through the online registration portal to participate in the “Activity”.
